Tooth resection

A surgical procedure that removes only the damaged areas of the root, leaving the healthy tissue intact is called a root resection. Tooth resection - what it is

During this surgical procedure, a specialist performs re-endodontic treatment of the root canals and replaces old fillings. A huge advantage of resection is the fact that the surgery can be performed even on the tooth under the crown. The surgeon is able to reach the root through the bone tissue.

The main purpose of this procedure is to remove the damaged part and not allow the infection to spread further. Generally, it is most common to resect anterior and double-rooted teeth on both the upper and lower jaw.

When tooth root resection is necessary

A lesion of inflammation caused by poor quality filling of the root canals is one of the indications for resection. When a patient is treated for pulpitis, or preparatory manipulation for prosthetics, dental canals must be filled along their entire length. If there are areas with insufficient filling, pockets of infection flare up in them. This leads to granulomas, periodontitis and inflammation of the tooth root tip. 

As for neoplasms, in this case treatment will be effective if the size of the cyst allows. The larger it is, the more difficult it is to treat. When medications are put in several times, it can, on the contrary, cause aggravation. If the cyst does not shrink within 12 months, the specialist decides to resect it. 

The main indication for resection is a lack of improvement after 12 months of endotherapy of the inflammation at the root tip.

Contraindications to resection

This procedure is not performed when:

  • obstruction of the root canals of healthy teeth;
  • paradontosis with severe exposure of the tooth neck;
  • paradontitis with tooth mobility.

Resection is also not performed in patients suffering from chronic diseases such as diabetes, heart failure, cancer, etc.

How the resection procedure is performed

First of all, the patient is X-rayed. With the help of this equipment it is possible to identify the size of the mass, its location relative to the anatomy, etc. 

Then the resection is performed according to the following algorithm:

  • injection with an anesthetic;
  • detachment of the gum tissue flap;
  • cutting off the area of the root affected by the infection;
  • retrograde filling of the root canals of the tooth.

The patient is then stitched, and the stitches are removed after 7 days. Resection is performed under local anesthesia, the surgeon cuts the gum and cuts off the root affected by the infection. The doctor can get to the root canal and fill it so that harmful bacteria cannot access it and re-infection does not occur. On the root side, a retrograde filling is done to speed up tissue repair. The incision is then sutured. 

This procedure has only one disadvantage - the anatomical integrity of the root of the tooth is violated. Because of this, its functional performance may be somewhat reduced. However, if you follow all the doctor's recommendations after surgery (which are presented below), it will ensure a long time of the tooth's service life, without any discomfort.

  1. To reduce swelling after the procedure, you should put something cold on your cheek.
  2. Avoid physical activity for 7 days.
  3. It is necessary to follow the doctor's orders clearly and take the antibiotics indicated by him regularly.
  4. Regular oral hygiene must be strictly adhered to. 
  5. Heavy loads on the tooth during the three months when food is being eaten must be avoided.

In order to prevent recurrence of inflammation, X-rays must be taken every 3 months.
