Sinus elevator
Sinus elevator is a light surgical procedure to increase the amount of bone in the upper jaw after a tooth extraction and before placing an implant. It is used when there is a lack of bone tissue in the jaw and also when the maxillary sinus cavity is close to the implant site.

- reduced traumatization;
- fear of surgical procedures.
If indicated and requested by the patient, the surgeon may exclude osteoplasty from the treatment plan. However, it is not always possible to place an implant this way.
Types of implant placement
In order to implant an artificial structure into the bone, several techniques are used. The situation is more complicated if there is an initial stage of bone reduction and osteoplasty is undesirable. In this case, the implantologist must choose a method based on indications.
- To place an artificial orthopedic structure inside the posterior wall of the maxillary sinus.
- Fix the prosthesis inside the frontal part of the bone, bypassing the maxillary sinus.
- Fix the implant in the distal portions of the maxilla where the bone thickness is greatest.
- Place a mini-implant.

All-on-4 is an implant technology, which is a metal structure and four prostheses. It is used when there is a lack of all teeth on the upper jaw, insufficient amount of bone tissue. The device allows the implants to be fixed tightly, with proper positioning.
How is the installation of the structure carried out?
- Two implants are implanted in the frontal area parallel on both sides.
- Two tilted prostheses are placed in the lateral regions, parallel to each other.
- A one-piece metal structure is placed on top of the implants. It makes the apparatus rigid, unites and secures the implants.
Before the system is created and placed, the patient is referred for a CT scan. It can be used to get information about the condition of the bone tissue, to determine the points of implant placement. The possibility and speed of engraftment of the prosthesis depend on the digital modeling.
The all-on-four method allows implantation without osteoplasty, with a high survival rate and good predictability of the treatment outcome.
Technology of basal prosthesis implantation
Basal implantation is a technology that allows implants to be securely fixed in the jaw arch even when there is minimal bone volume. The effectiveness of this method is due to the use of implants that are made in the shape of a cone and have a cylindrical thread. Thanks to this, the construction is easily and quickly placed even in the cortical plate of the bone (the hardest part).
The distance between the microthreads is wide, which improves the implantation process. During implantation, bone repair begins. The tissue fills in the areas between the screw threads, firmly securing the prosthesis.
Indications for basal implant technique:
- fragmentary, complete reconstitution of the dentition;
- significant loss of bone tissue;
- advanced periodontitis, periodontal disease;
- inability to perform a sinus elevator due to contraindications.
Basal implantation does not provide for osteoplasty. The prosthesis is placed deep into the bone tissue, where atrophy is excluded: cortical and basal, zygomatic bone.
Denture implantation without sinus elevator - where to go
Tooth implantation is a surgical procedure, so the ability to minimize the amount of manipulation is a big plus. Placing a tooth without bone augmentation has a number of advantages.
- Treatment time. Manipulation time is significantly shorter without osteoplasty. More often than not, a single-stage implantation of the prosthesis is performed.
- Number of manipulations. Surgical treatment is performed according to a small protocol - there is no bone grafting, no opening of the gingiva over the prosthesis during the placement of the abutment.
